PLEASE NOTE**Under travel, if you need your player to travel home with you after a game- PLEASE get your email request in to Coach McKee by Wed 4 PM (for Friday games) (Thursday games will need to be in by Tue 4 PM), only use email smckee@amphi.com
The EMAIL MUST INCLUDE...
- All District paperwork filled out AND Signed. (See attachment)
-Email must say who will be taking the player home after the game
-AFTER the game, Player MUST text a picture of themselves with the person they are leaving with to their Head Coach
It is an expectation that players ride the bus both there and back (THIS FORM IS only for occasional exceptions AFTER games)
ALL PLAYERS MUST TRAVEL WITH THE TEAM TO THE GAME. No Exceptions
